Chromium, biotin, vitamin E and zinc may help to increase insulin sensitivity.

Q.

My husband has been unable to control his blood sugar level. He is now ranging in the 400-500 despite the addition of regular insulin to the previous 70/30 doses. His doctor believes he is becoming insulin resistant, but as he is also in renal failure (10-15 percent kidney function remaining), he is reluctant to add more drugs to his system. He is OK with trying to find herbal remedies that are metabolized by the liver rather than the kidneys. Would bitter melon be helpful in this case? Any other suggestions would be appreciated.

A.

First of all, it is important that your husband follows the instructions of his physician for controlling the blood sugar. Insulin resistance may require the addition of other medications or different types and administration regimens of insulin in order to keep the blood sugar level out of a dangerous range. Besides the risk of diabetic coma, not controlling the blood sugar will also lead to further decline in kidney function.

Your husband also needs to closely follow diabetic dietary guidelines- both to control blood sugar and to avoid further taxing his kidneys. Diabetics, especially ones with hard to control blood sugar must avoid all forms of simple, processed and concentrated carbohydrates. That includes all sugars, many fruits and fruit juices, high starch vegetables like potatoes and beets and other high starch foods like rice and bread. While a higher protein diet can be helpful for some diabetics, in the case of kidney failure too much protein is harmful. Ask his doctor and the dietitian for advice on the foods he should be eating.

There are a number of nutrients that may help to increase insulin sensitivity including chromium, biotin, vitamin E and zinc. Bitter melon does help to control blood sugar levels but it is not clear whether it helps to increase insulin sensitivity. Before starting any nutrient supplements or herbs you should consult with his doctor since there may be interactions with his medications or contraindications with his health conditions.
